Report a safety incident

Do not use this online form to report emergencies. If there is an emergency, please immediately call 911 and then report it to the Community Safety Unit by calling 416-921-2323 .

If you have any information about a serious crime, please report it anonymously to Toronto Crime Stoppers online or call Toronto  Crime Stoppers at 416-222-8477 (TIPS). You will never have to give your name or testify in court, and Toronto Crime Stoppers does not have call display.

How to use this form

Use the form above to report any non-violent crime or suspicious activity that does not present a danger to any person. Fields marked with an asterisk * are mandatory—you must fill them in. The form will be submitted to Toronto Community Housing's Community Safety Unit.

How to complete each section of the form

First name

Enter your first name in this field.

Enter your last name in this field.

Date/time of incident:

Use the calendar to select the date of the incident. The single arrows <> at the top of the calendar allow you to go backward (<) or forward (>) by one month. The double arrows << >> allow you to go backward (<<) or forward (>>) by one year. Time is recorded in intervals of 15 minutes. Round up or down to the nearest time. You can provide the specific time in the Tell us what happened box.

Address or where the incident happened:

Write the address of where the incident happened and/or any other details that would help us locate where the incident happened.

Type of incident:

Choose the most accurate description of the incident from the drop-down choices available. Please note that you should not use this form to report emergencies or the following incidents :

  • theft or loss of property valued at more than $5,000

  • a violent crime

  • suspicious activity that presents a danger to the community or a person
